Social Prescribing
Social Prescribing provides a non-medical intervention to help tackles issues such as loneliness, low level mental health, poor wellbeing, and social isolation.
Social Prescribing often begins with a referral from a GP, hospital, charity or other support agencies and Charities, although self-referrals can be taken as well as referrals from friends & family.
Social Prescribing Link Workers listen to people and try to understand their situation and what matters to them. They then connect that person to organisations and information that can help.
Individuals will be connected to local community activities and groups.
Participation can improve overall wellbeing whilst helping reduce the demand for medical services.
